Pharmacist

Baljit Ahitan

The primary role of the pharmacist is to promote safe, appropriate and cost-effective drug treatment.  The pharmacist will provide information and advice on medicines to all staff involved in the prescribing and administration of drugs, as well as to patients and carers.

The pharmacist will review all medication prescribed for CF patients. This includes ensuring an accurate and complete drug history on admission, advising on the choice and appropriateness of therapy, monitoring of adverse reactions and therapeutic drug level monitoring. This is particularly important for CF patients as they are usually prescribed a number of medications, and also the doses employed may be greater than for the general population.

The CF pharmacist is able to liaise with primary care colleagues regarding new therapies and medication changes. They are closely involved in the procurement of medication (including unlicensed and named patient items), as well the provision of home intravenous antibiotics - whether this is through the hospital pharmacy department, or through contracting with a Homecare Company.

The pharmacist will also have an input into the writing of CF drug policies and guidelines. Adherence to these guidelines will be monitored through the routine screening of all prescriptions.
